We'll send your new patient intake documents via email or text link. This allows a therapist to review your case and get a head start on your treatment plan of care. If you are unable to fill this our electronically, please arrive at least 15 minutes early with your insurance cards and driver's license.
You'll sit down one-on-one with your assigned therapist for an initial evaluation. They'll ask about your history, what's been bothering you, and what you want to get back to doing. This is a conversation, not an interrogation.
Based on your evaluation, your therapist will walk you through a personalized treatment plan — what you'll be working on, how often you'll come in, and what you can realistically expect from the process.
Your treatment plan is built around getting you back to the things you love. Show up, do the work, and trust the process — your therapist will be with you every step of the way.
